Please submit the following documents via email (info@divorceandqdro.com) or fax ((843) 686-2476). These materials are necessary to begin the review and preparation of your QDRO:
A copy of a recent account statement for the retirement plan in question (does not need to be the most current), if available
By submitting this QDRO request, you acknowledge that you are requesting the preparation of a Qualified Domestic Relations Order (QDRO).
If, during our review and research, it is determined that a QDRO is not required to divide the retirement account, a partial refund of $400 will be issued. A $100 administrative fee will be retained to cover the time and effort spent on initial research and case evaluation.
After you submit this form, it will be sent directly to our team. Please note that processing will not begin until payment has been received.
Upon submission, you will be redirected to a webpage where you can download payment instructions, including options for paying via PayPal or check.
You will receive payment reminders until the QDRO fee is paid in full.
